3RS A/V project: A reliability and validity study of the revised Rupture Resolution Rating System v2022

The importance of rupture repair for the patient-therapist alliance has been demonstrated repeatedly but underlying processes require further investigation. For this, the Rupture Resolution Rating System (3RS) is the best observer-based measurement available. Recently, it has been revised (version2022), but its application to videotaped psychotherapy sessions is recommended. Therefore, this study aims to a) establish the 3RSv2022’s interrater-reliability and construct validity in its application to audio-recorded psychotherapy sessions and b) validate 3RSv2022’s in order to facilitate the use of this measure for a wider range of psychotherapy process research. This will be achieved by directly relating 3RSv2022 ratings a) of video and audio recordings of respective psychotherapy sessions and b) with self-report alliance-measures. Targeted sampling will be performed through control-charting available post-session alliance scores to identify low and then high alliance session, functioning as proxies of rupture and repair. Two sessions from mid-phase of treatment will be selected and coded using the 3RSv2022 for a total of N=46 patients and N=98 sessions across four collaborative sites. By collaborating with the University Trier, University Kassel, University Klagenfurt and Mount Sinai Beth Israel, this project will be able to incorporate a diverse sample of patients and therapists from Germany, Austria, Switzerland, and the USA, with treatments including cognitive-behavioral, psychodynamic and integrative-relational psychotherapy. German and English sessions will be coded by teams in Berlin, Trier, and New York. Secondary analyses will consider any differences by geographical location, treatment modality, and spoken language. This study will provide an important contribution regarding measure development in psychotherapy process-outcome research.
